Eve Packwood is the appointed Data Controller and Data Processor for your data, in regards to your participation in physical activity classes with Eve and your data is collected in compliance with UK/EU GDPR Regulations. You can read more about your rights here

DATA YOU SHARE WITH ME WHEN YOU CONTACT ME /BOOK A CLASS ON THIS SITE

When you send me an email through the contact form, or book a class through the booking page on the website, you will provide me with your name, email address, telephone number and your question/s.

This information is stored in this password protected site and/or sent to my email address: zumbacornwall@yahoo.co.uk

I will use this information to:

  • Confirm your booking if there is a space available when you book

  • Get in touch to let you know a space has come up if you are on the waiting list.

  • Reply to your questions by email, phone or text you – which ever you have requested in your message. If you have not specified, the default contact method will be by email.

DATA YOU SHARE WITH ME WHEN YOU ATTEND A CLASS

When you attend your first class, I will ask you to complete a quick registration form - a Physical Activity Readiness Questionnaire or PARQ for short, which asks for two key areas of information:

Part 1

  • Your name (so I know who you are…. Hello there!)

  • Date of Birth (useful for the emergency services)

  • Telephone number (so that I can keep you informed about class cancellations/updates*)

  • An Emergency contact name number (useful for the emergency services)

  • Your email address (so that I can keep you informed about class cancellations/updates*)

 

*From time to time your contact details may be used to inform you of special events or class information other than cancellations/updates. You have the right not to be informed of such and  you have the right to request change by emailing zumbacornwall@yahoo.co.uk.

 

I do not store this PARQ information electronically. You may ask me at any time, to remove the personal data you have previously provided on your form. Please note you will still be required to sign a new copy of your PARQ form and complete part 2, to confirm your acceptance of the terms of taking part in the class. 

 

Part 2

Your form will also ask you to answer some basic questions about your health and fitness and to sign to say you understand the risks involved in taking part in the class. This is a very standard practice in the health and fitness industry, to make sure that as your instructor, I can offer you relevant information about the class in relation to the answers you provide.

 

Please note that this form and the advice you receive from me when you complete it, is NEVER a substitute for a consultation with a medical caregiver/GP.   I am not medically trained and if you are at all unsure about your suitability to attend this (or any other exercises class) I expressly advise you to check with your medical caregiver before you attend.

 

WHAT HAPPENS TO MY PAPER FORM?

This is stored securely. You have the right to request to see this form.

 

WHAT HAPPENS TO MY CONTACT DETAILS?

Your name, emergency contact information and date of birth will be transposed into a password protected document that is stored on a password protected computer and mobile device –  these passwords are known to the instructor only. A copy of this document is available to the device during the class, in the event of an emergency.

 

You have the right to see information you have provided me with, that pertains to you only, on this document.
