Before you attend your first online class, you will be required to complete a quick registration form - a Physical Activity Readiness Questionnaire or PARQ for short, which asks for two key areas of information:

Your contact information

Basic questions about your health and fitness to determine your suitability for taking part in the activity and your confirmation of the terms of participation in the class.

C)  DATA COLLECTED WHEN YOU ATTEND A STUDIO BASED CLASS*

The data is stored by Wix.comLtd  on their secure server, and passed to evesclasses@gmail.com - in both cases accessed by a password protected portal - password known only to the Data Controller.

Online PARQ data

The online PARQ form is hosted by Jotform Inc who are the Data Processors for the data you submit when you complete an online PARQ.  You can view their privacy policy at  https://www.jotform.com/privacy/  The data is transferred to by Eve Packwood who is the Data Controller for this data. The data is stored on their secure server, accessible via a password protected portal - password held by the Data Controller.   Data is downloaded and removed from the sever within 28 days of submission and transferred to a password protected device accessible only to Eve Packwood.

Paper PARQ data
This is stored securely. You have the right to request to see this form.
Your name, emergency contact information and date of birth you provide on this form will be transposed into a password protected document that is stored on a password protected computer and mobile device –  password known only to the Data Controller.  A copy of this document can be accessed on the mobile device during the class, in the event of an emergency.
